AGREEMENT TO REVISE PAYMENT SCHEDULE OF NOTE
This agreement of [date of agreement] is in reference to a loan, which is evidenced by an instrument dated [date of instrument], made by [Name of maker], of [address of maker], [city of maker], *state of maker], and payable to the order of [Name of payee], of [address of payee], [city of payee], [state of payee].
The instrument is secured by a [mortgage or deed of trust], dated [Date of Mortgage or deed of trust], on the following property: [description of property]. The [mortgage or deed of trust] is recorded in the office of the [County Recorder], of [Name of County] County, [Name of state], in the [registry book] at page [Page Number] of volume [Volume Number].
For value received the payment schedule of the instrument described above shall be changed in the following manner: [Change of instrument].
This agreement is only a revision and not a novation. Except as provided in this agreement, all the terms and conditions of the instrument and the [mortgage or deed of trust] described above shall remain in full force and effect.
